Ok, here is the format that was posted by evoth.
1. What you would like to be called:
Hmm..I don't mind really. Out of character I am usually Kez, Kezzer, Kelly or even just plain K.
2. What you tend to roleplay:
Likes: Vampire, Gifted school, Teen DNA and I love Teen Assassin, something people don't usually do but is really good. I'm basically open to many different things,except if it's on my dislike list.
Dislikes: Anime, Animal (Wolf, Horse, egg hatching etc) and anything too whitelight fantastical.
3. Do you have any limits(romance, violence, swearing)?:
Not particularly, I don't swear but have no problem with others swearing. Romance is fine with me in context as is violence.
4. How long have you been roleplaying for?:
Hmm....that's a hard question. I only really got into it in the last year or so when i found out how amazingly fantastic it is.
5. A sample of your roleplaying. An intro will do. Say what genre it is.
This is my Teen DNA intro
Pain. That was the first thing he felt. Not the sharp, cutting pain but a dull throb that permeated throughout his body.
He concentrated on the core of the pain, concentrated on it until it ebbed away slightly- enough to think more clearly…
His back, he decided hurt the most, it felt strange…. Really heavy and aching…He put the thought to the back of his mind and focused all his strength to the front of his head. He opened his eyes,
squinting slightly in the dim light. Everything was fuzzy but he could make out the cold grey concrete wall he was
slumped against. As things slowly began to clear, he could make out what the silvery thing in front of him was.
Bars…I’m in a cage…why?

I didn't know which intro to use, probably could have chosen better
